If you've been taking prescription progesterone, don't let the high dose of the prescription confuse you. The steady progesterone levels provided by Simply Progesterone usually yield superior results at much lower doses than your previous prescription.
Finding your optimal dose.
- Begin with a low dosage: When first starting to use molecular progesterone serum, it's recommended to start with a low dosage and gradually increase over time. This will help you avoid any potential side effects and ensure you get the most benefits from the serum.
- Dosage: For women, start with 2-3 drops twice daily (12-19 mg total). For men, start with 1-2 drops twice daily. Rub the serum gently into one of the areas of the skin listed below until it's thoroughly absorbed. (See below.)
- Allow 2-6 weeks at this dosage level before increasing or decreasing the dose. Your body needs time to adjust to the new hormone levels, and you need time to see how you feel.
- Adjusting your dose: after 2-6 weeks, assess how you are feeling. If you feel the need, this is the time to increase or decrease the amount of Simply Progesterone you use by 2-3 drops.
- Every person is different and biologically unique. Experiment with higher and lower dosages every 2-6 weeks until you feel optimized.
- Use consistently: For best results, it's important to use Simply Progesterone serum consistently over time. This means using it as outlined below for your life stage.
Progesterone Dosage Range Based On Life Stage:
- Perimenopausal Regular Cycle: up to 200 mg per day. (If menstruating regularly, cycle the progesterone by using it for 12 to 14 days during your cycle's second half, typically days 14 through 28. If your cycle has become irregular, follow the guidelines in section 2.) Note: Dosage requirements can fluctuate during this early phase. If you are feeling overly sedated, reduce the dose. If your symptoms reappear, increase the dose.
- Perimenopause Irregular Cycle: up to 150 mg per day. Gradually adjust the dose down from perimenopausal levels to the minimum needed to keep symptoms in remission. As your cycle changes, so do your hormonal requirements. No more cycling is required. (Once menstruation becomes irregular, progesterone requirements change. Switch from cyclical use as outlined in section 1 and start with continuous use.)
- Menopause Cycle Has Stopped: up to 100 mg daily. Use continuously, no need to cycle. Gradually adjust dose down to the minimum needed to maintain symptom remission.
Progesterone Application Locations
Apply to soft inner skin and rotate application locations to minimize tissue saturation. All of our products are for topical use only.
Best locations for systemic absorption: Progesterone can be applied to the face, inner arms, upper chest, inner thighs, and lower abdomen. Avoid direct application to the breast to prevent progesterone buildup in breast tissue.
Best location for uterine protection: The best application location that focuses progesterone concentration in uterine tissue is the genitalia: vulva, inner & outer labia. This zone preferentially absorbs hormones and does not need to be rotated, unlike the other skin types listed above.
- Gum and Oral use is not advised as these routes promote the highest conversion of progesterone to allopregnenolone and produce the most side effects. OTC hormones are legal for topical and cosmetic use only.
- Transdermal application yields the highest and most steady blood levels of active progesterone.
Avoid the following locations:
- Breasts
- Testicles
- Eyes Mouth & lips.
Breasts and testes do not distribute progesterone well into the general circulation; the hormone can build up to unhealthy levels.
Simply Progesterone is not intended to be a sunscreen. UV light can break down progesterone, so it is best to avoid areas that will be in direct sunlight that day. Simply Progesterone does not cause skin photosensitivity in most people.
Rotate body locations.
Applying Simply Progesterone to a different body part each day is also best. Rotation helps keep the skin evenly healthy and not overloaded in any one area with progesterone.
Monitor for results.
Finally, monitoring your progress over time is important to ensure you get the intended benefits from the protocol. Our brains tend to forget subtle changes in symptoms, so it's best to keep a journal that you can refer back to.
Should you cycle off of progesterone during your menstrual cycle?
During my clinical practice, I found that this question is very individual. Some feel better taking a break each month, while others feel better continuing their regular dose uninterrupted. This is more the case during the perimenopause stage. I think it boils down to a few factors: your individual sensitivity to a sudden lack of progesterone supplementation and the total daily dose being taken. Ultimately, you should experiment and see which supplementation pattern best suits your body. Generally speaking, though, the higher the daily dose, the more likely you are to benefit from a break. Since this protocol aims to find the lowest effective dose that relieves symptoms, cycling off is usually unnecessary. Your body is going to continue to produce its own natural ebb and flow of sex hormones without you having to alter your daily routine. While starting out, focus on building up and finding your best dose, and don't worry about cycling off during menstruation. Only after you have found your therapeutic range should you experiment by taking a break, stopping progesterone supplementation at the first sign of your cycle, and resuming again 5-7 days later. Journal how you feel and decide whether to do it again next month.
What to do if you are not seeing the desired results from progesterone-only therapy.
Only 20% of women see complete optimization with progesterone-only therapy. Adding a form of estrogen therapy can increase the success rate to 90%. This is because estrogens decline with progesterone and can produce similar symptoms. Combined progesterone, estrogen therapy might be right for you if the any of the following conditions apply.
- If you are experiencing progesterone side effects before reaching a progesterone dose that relieves your symptoms.
- If you feel that you have reached an optimal progesterone dose but are still experiencing symptoms.
- Vasomotor Symptoms (the most classic indicator): Ongoing hot flashes, Night sweats, or Flushing episodes.
- The most indicated product for this symptom set is Simply Bi-Est, especially if you are perimenopausal or early menopausal. Estriol has been shown to help, but not as consistently for vasomotor symptoms.
- Progesterone alone may help with sleep, but usually doesn’t fully control vasomotor symptoms, since they’re strongly linked to low estrogen or fluctuating estrogen.
- Cognitive & Mood Symptoms (estrogen-sensitive): Persistent brain fog, Memory issues, Difficulty concentrating, Low mood, or depressive symptoms not improving.
- Sleep Disturbance (if still present): Trouble staying asleep, especially if tied to night sweats, Non-restorative sleep despite progesterone.
- If sleep issues are driven by vasomotor instability, estrogen is usually needed.
- Genitourinary Syndrome of Menopause (GSM): Vaginal dryness, Pain with intercourse (dyspareunia), Recurrent UTIs, Urinary urgency or irritation, genital itching, internal discomfort while sitting or with movement, microtearing and spotting from thinning of the genital skin or vaginal wall.
- These are highly estrogen-dependent tissues. Progesterone alone won’t fix this symptom set.
- Simply Estriol is the most indicated for this symptom set unless you are perimenopausal, then use Simply Bi-Est.
- Musculoskeletal Symptoms: Joint aches or stiffness, or increased perception of pain.
- Estrogen plays a role in inflammation and joint health.
- If you are experiencing this symptom set, Simply Bi-Est is the most indicated, as estriol will not have a systemic impact on joints.
- Skin, Hair, and Tissue Changes: Dry skin, Thinning hair, Loss of skin elasticity.
- Again, largely estradiol-mediated, so Simply Bi-Est is the most indicated.
- Sexual Function Changes: Low libido (especially if linked to dryness or discomfort), or reduced arousal.
- Often multifactorial, but estrogen deficiency is a major contributor.
- Perimenopausal use Simply-Bi-Est, and if menopausal or later, use Simply Estriol.
- Testosterone deficiency can also play a role.
Important Caveat
The decision to add estrogen depends on:
- Personal and family history (e.g., breast cancer, clotting risk)
- Cardiovascular risk
- Time since menopause
So this is not purely symptom-driven, but symptom persistence is a key clue.
Estrogen Dosage Guidelines
- Always use the lowest effective dose.
